If your child is going to be coming in without parents, we need permission to treat and a summary on the child’s health

Access and Communication:
Our practice would like to be available when you need us. Please ask about any special communication or access needs that you or your children have.
We have your needs in mind when scheduling well care appointment, follow up appointments, and sick visits. Same day and extended hours are available by contacting our office staff during regular office hours to schedule.
When your child has tests or procedures done you will get the results by telephone if abnormal. You can always call to get any results
If you are referred to a specialist with your permission we will share information with them and with you in a timely manner.
